Cyproterone Acetate

Cyproterone acetate is an anti-androgen. It also has progestational properties. Through its weak anti-gonadotropic action, cyproterone inhibits androgen-receptor interaction and lowers serum testosterone. It is the only anti-hormone that causes complete androgen blockade when used alone.

Cyproterone is indicated in-

  • severe hypersexuality
  • sexual deviation in males
  • prostatic cancer

Typically, treatment begins with 1 tablet Androcur 50 mg twice daily, after breakfast and dinner. Individual treatment duration for cyproterone acetate should be determined. After achieving a satisfactory result, the therapeutic effect should be maintained at the lowest possible dose. When changing or discontinuing cyproterone acetate, it is best to do so gradually.

Commonly associated side effects are-

  • decreased libido
  • erectile dysfunction 
  • reversible inhibition of spermatogenesis
  • weight gain
  • fluid retention
  • depression
  • dyspnea
  • Direct hepatic toxicity, including jaundice, hepatitis, and hepatic failure, has been reported in Cyproterone patients.
  • Cyproterone has been linked to the development of benign and malignant liver tumors, which can result in life-threatening intra-abdominal hemorrhage.
  • Patients taking Cyproterone have reported thromboembolic events, though a causal relationship has not been established.
  • Meningiomas (both single and multiple) have been reported in association with the use of cyproterone acetate, primarily at doses of 25 mg and higher.
  • Adrenocortical function should be monitored on a regular basis during treatment, as preclinical data suggest a possible suppression due to Cyproterone's corticoid-like effect at high doses.
  • Shortness of breath may occur during high-dose Cyproterone treatment. This could be due to progesterone and synthetic progestogens' stimulatory effect on breathing, which is accompanied by hypocapnia and compensatory alkalosis and is not considered to require treatment.

Contraindication

Contraindicated in patients with hypersensitivity to cyproterone or to any of the excipients

Cyproterone is contraindicated in-

  • Liver dysfunction
  • Jaundice
  • Malignant tumors
  • Severe diabetes
  • Severe depression
