Tapentadol

Tapentadol is an Opioid analgesic. Tapentadol is a centrally acting analgesic that works through two mechanisms - mu-opioid receptor agonism and norepinephrine reuptake inhibition. It provides pain relief by binding to mu-opioid receptors in the brain and inhibiting the reuptake of norepinephrine, enhancing descending inhibitory pathways to reduce pain perception.

 

  • Tapentadol is indicated for the management of moderate to severe acute pain in adults.
  • It is used when alternative treatments are inadequate or not tolerated.
  • The dosage of Tapentadol should be individualized based on the patient's pain severity, response, and tolerability.
  • The recommended starting dose is 50 to 100 mg orally every 4 to 6 hours as needed for pain.
  • The maximum daily dose is 600 mg.
  • The dose may be adjusted based on the patient's needs and response, but caution should be exercised to avoid overdose or adverse effects.

  • Nausea
  • Vomiting
  • Dizziness
  • Constipation
  • Headache
  • Somnolence
  • Pruritus
  • Sweating
  • Fatigue
  • Dry mouth

 

 

  • Tapentadol has a risk of respiratory depression, misuse, abuse, addiction, and overdose. It should be prescribed and administered with caution, especially in patients with a history of substance abuse or addiction.
  • Prolonged use of Tapentadol may lead to physical dependence and withdrawal symptoms upon abrupt discontinuation.
  • Tapentadol should be used with caution in patients with impaired respiratory function, liver or kidney disease, or a history of seizures.
  • Concurrent use of Tapentadol with other central nervous system depressants, including alcohol, may potentiate respiratory depression and sedation.
  • Tapentadol may impair mental or physical abilities required for driving or operating machinery, and patients should be advised to avoid such activities until they know how the medication affects them.
  • Seizures:  Use with caution in patients with a history of seizures.
  • Serotonin Syndrome: Potentially life-threatening condition could result from concomitant serotonergic administration.

 

Contraindication

Tapentadol is contraindicated in-

  • Hypersensitivity to any component of the medication  
  • Concomitant use with monoamine oxidase inhibitors (MAOI) or use within 14 days such as- 

 

Tapentadol is contraindicated in the following conditions-

  • Impaired pulmonary function (significant respiratory depression, acute or severe bronchial asthma or hypercapnia in unmonitored settings or the absence of resuscitative equipment)
  • Paralytic ileus
